Professor Joan Lalor
Professor of Maternal Health, School of Nursing & Midwifery
Biography
Prof Joan Lalor is the Chair of Maternal Health in Trinity College Dublin. Joan was awarded a BNS, MSc, MA, PGDip in Clinical Health Sciences Education, a PGDip in Statistics and a PhD from TCD and a Diploma in Legal Studies and a Barrister at Law degree from the Honorable Society of the Kings Inns Dublin. Joan was elected as a Fellow of Trinity College Dublin (FTCD) in April 2022, a recognition by the University of Scholarship or research achievement of a high order.
Joan designed and led a pan-European COST Action CA18211 "DEVOTION "as Action Chair from 2019 to its successful completion in December 2023. This pan-European network of 33 countries and 300+ members aimed to 1) consolidate and disseminate current evidence and coordinate a joint effort to seek ways to prevent, minimise and resolve birth-related trauma, and to optimise emotional and psychological outcomes for parents and families and 2) accelerate the translation of that knowledge into best practices that can be shared across Europe to reduce the societal and economic burden arising from birth-related negative/traumatic experiences. From Devotion- a new COST Action "TREASURE" www.ca22114.eu. has emerged, with many of the members from DEVOTION joining to continue that work. Joan is currently one of Ireland's management committee members on COST Action "TREASURE", and is also leading a Working Group on the development of a Review on Maternal Perinatal Stress.
Joan has recently been awarded HORIZON Europe funding for the Healthy Adaptation to Pregnancy, Postpartum and Parenting (HAP3) project which aims 1) to examine risk and protective factors, and mechanisms of obesity and mental health problems.
In addition Joan has recently been awarded a grant to explore consent for interventions in later pregnancy and birth.
Joan's research contribution in Maternal Health is organised around the following main areas: i) Maternal and Fetal Health including Birth Related Trauma and Complex issues in Childbirth, ii) Optimising Maternity Services to improve maternal and neonatal health outcomes iii) Research Methodology, Healthcare Ethics and Law. Joan is also centrally involved in a number of collaborative interdisciplinary projects across Health, Education and the Humanities. The IRC CORALE project aims to investigate, for the first time, the operation of the right to conscientious objection in abortion services in Ireland and to develop a coherent framework for situating conscientious objection within a system of access to abortion services in a manner that is proportionate, effective and legally robust. Our preliminary findings have been shared with and incorporated into the recommendations of the Termination of Pregnancy Legislation Review Group recently sent to Government for review/approval.
Joan has been awarded over 7.22m in funding as PI or co-applicant from a range of funding bodies. To-date, she has published 192 peer reviewed publications and other outputs in high-impact journals/conference proceedings etc. Joan has also held a number of academic and leadership roles in the school such as course coordinator and Directorships in both Postgraduate Teaching and Learning and in Research, holding committee positions at college level. Joan launched and implemented the School of Nursing and Midwifery PhD Scholarship programme in March 2020 which is currently supporting 12 doctoral students. Joan continues to teach and supervise across undergraduate and postgraduate programmes. Joan also worked as a college tutor providing pastoral support to students across faculties from 2008-2022 and has been a member of the college Academic Appeals Committee since 2008. Joan has also served on the Faculty of Ethics Committee as a member and co-chair for seven years, and has acted as Chair of THEConf- Committee 2019-2020.

- Maternal Perinatal Stress and Adverse Outcomes in the Offspring: Maximising infants"development (TREASURE)
- Summary
- This COST Action is a follow on Ac tion from CA 18211- Devotion focussing further on the transgenerational consequences of maternal stress on offspring and the family. High levels of maternal perinatal stress are associated with negative effects in the offspring. The adverse impact maternal stress can have in the infants"s health is 1) in the short term (prematurity, low infant birthweight); 2) the long-term (neuroinflamation, autism); 3) the very long-term (transgenerational effects). Previous studies have reported maternal stress can have transgenerational consequences. During pregnancy, high levels of maternal stress can cross the placenta and reach the fetus. Mediators responsible for the impact of maternal stress in the developing fetus include cytokines, tryptophan, cortisol, cathecolamines, reactive oxygen species, oxidative stress and microbiota. These mediators, along with epigenetic mechanisms, are involved on the adverse consequences high levels of maternal stress can have in the offspring. In order to improve fetal development and boost infant"s health throughout their lifespan, the TREASURE project aims to consolidate a multidisciplinary and international network of scientists, clinicians, students, stakeholders, Non-Governmental Organisations (NGOs) and Enterprises to achieve impact through a three-fold main objective : 1) discovering, reviewing and disseminating scientific evidence on minimize, reduce and prevent the impact of maternal perinatal stress on fetal development, and to improve psychological, medical and neural development in the offspring during their life-span; 2) bridging knowledge, evidence and experience between scientific disciplines, and bringing international research groups together to increase knowledge exchange between countries. 3) forming international coalitions to efficiently translate scientific knowledge into clinical guidelines and best practices across Europe to improve the health of children, and reducing economic cost appearing from high levels of maternal perinatal stress.

- The CORALE Study - Conscientious Objection after Repeal: Abortion, Law and Ethics
- Summary
- In May 2018 the Irish people voted to repeal the 8th Amendment to the Constitution and paved the way for the rollout of abortion services in 2019, heralding a radical change in clinical practice as well as in law. A major challenge that has emerged in abortion provision is how to accommodate the right to conscientious objection while ensuring equitable access to abortion services. The right of conscientious objection allows a person refuse to engage in a particular task by reason of their personal moral objection to it. The Health (Termination of Pregnancy) Act 2018 provides for a right to conscientious objection. Anecdotal evidence suggests that right is quite widely relied upon, with the result that access to abortion may be limited in some locations. However, the right does not extend to certain persons who may strongly object to abortion, such as health care assistants, hospital administrators, and pharmacists. The objective of the CORALE Study is to investigate, for the first time, the operation of the right to conscientious objection in abortion services in Ireland and to develop a coherent framework for situating conscientious objection within a system of access to abortion services in a manner that is proportionate, effective and legally robust. To investigate this complex question, an interdisciplinary approach is inherently necessary, because it seeks to understand how a legal right operates in the reality of clinical practice. The Project Team spans the disciplines of law, midwifery and ethics. Building on a sophisticated theoretical framework, the Project will employ interviews, focus groups and surveys with those involved at the frontline of abortion provision. The Project will produce evidence and recommendations that will inform abortion law and policy in Ireland, but also generate important contributions to international scholarship and policy debate on conscientious objection in abortion services.

- Women's Experiences of Psychological Trauma during Childbirth
- Summary
- This project involves bringing together international experts to contribute to this edited book on Childbirth Related Trauma. The focus of this book involves a new and emerging area, the field of perinatal or birth-related trauma which is relatively unrecognised. It is quite recent that acknowledgement has been given to the possibility that Birth can be a traumatic experience even when the life/health of the mother and infant are not significantly challenged. Intergenerational trauma is also an emerging field.

- CA18211 - Perinatal Mental Health and Birth-Related Trauma: Maximising best practice and optimal outcomes. The DEVoTiON (DEliVery-related Trauma Outcomes Network)
- Summary
- Unlike other sources of trauma, perinatal or birth-related trauma is relatively unrecognised. Evidence suggests up to 30% of women describe their birth experience as traumatic and experience some symptoms of intrusion, avoidance or hyper-arousal. Meta-analyses show post-traumatic stress disorder (PTSD) affects 4% of women after birth and up to 18% of women in high risk groups. Rectification of this situation is essential. In 2016, 5.11 million babies were born in Europe, indicating that up to 1.5 million women may have had sub-optimal birth experiences and over 200,000 may have developed PTSD as a result. Developmental research has firmly established that the quality of infant-parent relationships is a critical factor in early and later childhood development, consequently, a family-centred approach to any investigation of birth-related trauma is critical, as trauma can be transmitted within the family system. Given the enormous economic burden it places on women, health systems, and particularly children, relatively small improvements in services to prevent, detect and treat this problem can benefit society significantly. The main aim of this Action is two-fold. The Action will establish an international multidisciplinary network of researchers, clinicians, NGOs and SMEs to 1) consolidate and disseminate current evidence and coordinate a joint effort to seek ways to prevent, minimise and resolve birth-related trauma, and to optimise emotional and psychological outcomes for parents and families and 2) accelerate the translation of that knowledge into best practices that can be shared across Europe to reduce the societal and economic burden arising from birth-related negative/traumatic experiences.

- INTERSECT An international Survey of Childbirth-Related Trauma
- Summary
- Being pregnant and having a baby is a time of huge physical, psychological and social changes for women. It is therefore a period of rapid transition and adaptation. Although the birth of a baby is viewed positively in most cultures, research suggests between 20 and 40% of women find childbirth traumatic and 3-4% of women may develop post-traumatic stress disorder (PTSD) as a result. Birth trauma and PTSD have a substantial impact on women and their families. Postpartum PTSD is highly comorbid with depression and fear of subsequent births, as well as reduced breastfeeding, poorer child development and strain on the couples relationship. The INTERSECT study The INTERSECT study works in collaboration with principle investigators across the world, with an emphasis on working with under-represented countries in South-America, Asia and Africa. The protocol has been publicly registered and is available here. Principle investigators in each country have the responsibility of conducting the INTERSECT survey with postpartum women in their country. The INTERSECT project will thus result in cross-cultural information on the prevalence of postpartum PTSD, as well as cross-cultural variation in the etiology and manifestation of childbirth-related PTSD worldwide. This research aims to study childbirth PTSD in an international context. Specifically to: Determine the prevalence of birth trauma and PTSD across countries and cultures. Determine differences in symptom presentation across countries and cultures. Determine the etiology of childbirth-related PTSD symptoms across countries and cultures. Prof Lalor is the principal investigator for Ireland working with doctoral student Ursula Nagle.

- Autonomy for Two - A Longitudinal Study of Women's Understanding of the Maternal and Fetal Dyad and its implications for maternal autonomy in post-repeal Ireland.
- Summary
- Maternal and fetal interests often seem to be framed opposing each other, even in the absence of a legal framework supporting fetal rights. This study sets out to explore how women approach decision-making where a perceived or real conflict between fetal and maternal wellbeing exists and how they navigate the moral-ethical space of their symbiosis. A grounded theory study is designed for this purpose using vignettes as an additional means of investigation. Conducted in Ireland where the legal status of the fetus has only recently changed, it is hoped that the emerging theory will help promoting maternity services in which maternal autonomy and -rights are safeguarded in the interest of both mother and baby.

- Improving the organisation of maternal health service delivery, and optimising childbirth, by increasing vaginal birth after caesarean section (VBAC) through enhanced women-centred care (OptiBIRTH)
- Summary
- This study addresses the widespread concern over rising Caesarean section rates throughout Europe, particularly routine CS following a previous CS, despite calls for increased vaginal births after Caesarean (VBAC). VBAC is associated with lower maternal mortality than repeat CS, less overall morbidity for mothers and babies, and is the preferred option for the majority of women. VBAC rates in Ireland, Germany, and Italy are significantly lower (29-36%) than those in the Netherlands, Norway, and Sweden (45-55%). The OptiBIRTH project will use a cluster randomised trial in Ireland, Germany, and Italy, with 15 clusters of 94 women in each, to increase VBAC rates from 33% to 53% through a complex intervention designed to increase women-centred care and women's involvement in their care. The multi-national, multi-disciplinary research team for this project developed from the ESF-funded workshop: "Promoting Normality in Childbirth" held in the School in March 2010, and from the COST Action, (IS0907: Childbirth Cultures, Concerns, and Consequences: Creating a Dynamic EU Framework for Optimal Maternity Care), which started in June 2010. The team is composed of 12 partner institutions from 8 countries, and includes service users, midwives, obstetricians, neonatologist, epidemiologist, sociologist, psychologist, bioethicist, health economist and a 'high-tech' small industry. - An exploratory RCT of a manualised nurse-led telephone support intervention for people who support/carers of People with Multiple Sclerosis (MN-TISS trial).

- This is an intervention study with 200 carers randomly assigned to one of two groups: the intervention or control group. Using a manual designed to enable the nurse specialist to provide proactive telephone support, the nurse specialist(s) will empower carers by enabling them to learn problem management skills. Carer outcomes are determined in terms of improving self-efficacy in problem solving and obtaining respite, preparedness to care, quality of life and reduced carer burden. For People with MS outcomes are determined by exploring their experience of being cared for by a carer who has received telephone support from the multiple sclerosis nurse specialist. - the study aims to explore the strengths and weaknesses of publicly funded Irish health services provided to the following groups of women: (a) Women with a visual impairment (VI) (b) Women with a hearing impairment (HI) (c) Women with a physical disability (PD) (d) Women with a mental health difficulty (MHD) (e) Women with an intellectual disability (ID) Objectives: a) To review Irish and international literature on the challenges facing women with disabilities in accessing health services during pregnancy, childbirth and early motherhood (i.e within the first 2 years) (b) To document and review current policy governing procedure and practice in service provision at :- (i) HSE level, (ii) hospital/maternity unit level, (iii) relevant professional body level for women with disabilities who need to access publicly funded Irish health services during pregnancy, childbirth and early motherhood (c) To provide a commentary on the state of such policy development in Ireland vis-à-vis the international situation d) To seek and discuss the views of the relevant service providers as to strengths and weaknesses of the services provided and their opinion on the development required (e) To ascertain and explore the views and experiences of women with disabilities, throughout the countryand across all socio-economic groupings, in relation to (i)the strengths and weaknesses they encountered in publicly funded Irish health services during pregnancy, childbirth and in the first two years of motherhood, (ii) their experiences of accessing General Practitioners (GPs), Midwives, Obstetricians (Obstets), Neonatologists, Paediatricians (Paeds), Allied Health Professionals (AHPs), Public Health Nurses (PHNs) and Public Health Doctors (PHDs), health care workers, family support workers, and (iii) the service they received from these people. - Recasting Hope. A longitudinal study of women's experience of carrying a baby with a fetal anomaly up to and beyond the birth.
- Summary
- Abstract Aim: The overall aim of this study is to explore and understand women's experiences of carrying a baby with a fetal abnormality up to and beyond the birth of the baby, from which recommendations can be made to optimise provision of care. Background: Routine ultrasound in pregnancy has received uncritical acceptance in pregnancy from both women and clinicians alike. However, due consideration has not been given to the psychosocial consequences of mass screening resulting in fetal anomaly detection in low risk populations. Methodology: A constructivist grounded theory (n=38) was carried out in 2004-2005 using repeated in-depth interviews before and after the birth. Thirty women chose to continue the pregnancy and eight women accessed termination of pregnancy services outside the state. Data were analysed using constant comparative analysis. The usual tenets of written informed consent were obtained. Findings: The conceptual framework of Recasting Hope emerged and represents the process of adaptation after fetal anomaly diagnosis. Recasting Hope is a theory of traumatic loss, and is idiosyncratic to the area of prenatal diagnosis. The process of adaptation is represented temporally in four phases 'Assume Normal', 'Shock', 'Gaining Meaning' and 'Rebuilding', whereby women are concerned with reconstructing meaning in the aftermath of an ostensibly negative event in order to resolve the inconsistency between the experience and previously held views of pregnancy and beliefs in the world in general. Conclusion: The process of adaptation after fetal anomaly diagnosis is complex. The framework of Recasting Hope is intended for use as a tool to assist health professionals through offering simple yet effective interventions in order to support women during this time. The data have shown that women need particular support during this crisis. Therefore consideration of the impact of a midwife practitioner role on the process of meaning reconstruction is recommended.
