- Predicting who will respond to stimulant medication in ADHD: A precision medicine approach using neurally informed computational models
- Summary
- This project uses precision medicine to improve treatment of childhood Attention Deficit Hyperactivity Disorder (ADHD). Methylphenidate is the first-line pharmacological treatment but there is wide individual variation in outcomes: 30% of children will have no clinically meaningful response. There is currently no validated objective, quantitative method for predicting who will ultimately respond. We propose to use neurally informed computational models derived from behaviour and electroencephalography to decompose medication effects on distinct neural mechanisms. Our industry collaborators, Cumulus Neuroscience, have developed dry-sensor electroencephalography headsets allowing users to obtain data at home. Their cloud-based software can automatically process large amounts of electroencephalographic data. We will recruit 300 medication-naive patients (ages 10-14) from ADHD services led by co-PI McGrath and our collaborators. Tasks will be gamified to maintain engagement. Interrogating this immense amount of data will be solved by using computational model parameters as inputs into machine learning algorithms. The goal will be to identify neurocognitive processes predicting who will respond clinically to methylphenidate. Two PhD students and a research assistant will be recruited. This project can significantly enhance quality of life of those with ADHD. The approach outlined here can be a new sector in the precision medicine field.

- Understanding the mechanism of action of methylphenidate in ADHD: A computational psychiatry approach
- Summary
- Approximately 1-in-20 children worldwide have Attention Deficit Hyperactivity Disorder (ADHD), a condition associated with disabling inattention, hyperactivity and impulsivity. These problems can manifest as poor inhibitory control (e.g., difficulty holding back impulsive actions) and atypical reward processing (e.g., failing to learn from negative outcomes). Poorly treated ADHD is associated with negative academic and socioeconomic consequences. This project aims to ultimately improve clinical management of children with ADHD. Methylphenidate, a stimulant medication, is used as the first-option pharmacological treatment for ADHD and often successfully reduces problem behaviour. Although Methylphenidate can be extremely effective, it does not work for every child. There is currently no `objective" way (e.g., blood test or brain scan) to measure if a child is genuinely responding to Methylphenidate. Instead, clinicians must rely on reports from parents and teachers, an approach that is problematic and that often leads to delays in optimising ADHD treatment. The absence of a biological test to quantify Methylphenidate response is primarily because we do not understand exactly how Methylphenidate changes behaviours to produce the known beneficial effects. This lack of knowledge is despite the very common use of this medication. This project will investigate the specific brain processes that are affected by Methylphenidate by recording brain activity and behaviour in children with ADHD (who have already been prescribed Methylphenidate as part of their clinical care) when they are on and off this medication. Brain activity will be recorded using two separate approaches, which are both non-invasive and routinely used in Trinity College Institute of Neuroscience: electroencephalography (EEG) and functional magnetic resonance imaging (fMRI). Brain activity data will be collected while children with ADHD are performing two computer-game like tasks. One task measures the child"s ability to hold back impulsive actions (inhibition) and the other assesses how they learn from positive and negative outcomes (reward processing). The data from the two tasks, the EEG recording and the fMRI scan will be analysed using advanced computer-modelling approaches to determine exactly how Methylphenidate changes behaviour. This project is important because if we can understand the brain mechanisms affected by Methylphenidate, we can ultimately develop a computerised measure that will allow clinicians to predict whether a child is going to respond to this treatment or not. Such a measure would allow clinicians to treat ADHD more effectively and would result in children with ADHD experiencing faster relief from symptoms.

- Developing a Patient and Public Voice (PPV) Program for the Global Brain Health Institute: At home and beyond
- Summary
- The concept of Patient and public voice (PPV; also known as `patient and public involvement", PPI) is designed to foster a meaningful and active partnership between the public and researchers. Active involvement may take the form of consultation, collaboration or user control (Boote et al., 2015). Examples include public involvement in prioritizing research, advising on a research project, assisting in the design of a project, or in carrying out the research (e.g., INVOLVE UK:2015, www.invo.org.uk/)researchers should involve patients and the public at all stages of the research cycle (INVOLVE. Briefing notes:2012), including formulating the research question. In countries such as the United Kingdom, PPV is a requirement to obtain research support from many funders (Department of Health, 2005), is necessary for ethical approval of human research (Iliffeet al 2013) and is likely to become a standard requirement for research. While to date fellows at UCSF have had the opportunity to interact with the Memory and Aging Centre"s (MAC) Family Advisory Council (FAC), this has not been a core part of the fellowship across both sites. There is both a need and an opportunity to enhance the learning experience in this space in a cross-site fashion. In this application, we propose a two-year development project to work with a Patient and Public Voice (PPV) panel for the two sites (TCD and UCSF) of GBHI, fully aligned with GBHI"s learning and research goals. Including PPV in the Global Brain Health Institute (GBHI) fellowship experience will foster the GBHI values of equity and promote person-centred research and practice.

- Our project uses a precision medicine approach to predict treatment response to stimulant medication in adolescents with ADHD. There are three novel aspects to this project relevant to this supplemental funding. First, we are developing gamified neurocognitive tasks to maintain engagement and interest of participants (essentially, applying principles from video game development to established laboratory tasks). Second, we will apply " at scale (~2,400 sessions) " sophisticated computational methods to the brain and behaviour data. Third, we are committed to making materials from this project accessible to the research community, adhering to FAIR principles. Dr Revenko is an experienced researcher with skills that perfectly match these three novel aspects, with experience in computational modeling and software development (including game development).

- Mindfulness-based interventions for substance misuse: Harnessing predictive models for successful treatment response and relapse prevention
- Summary
- Substance misuse is responsible - directly and indirectly - for approximately 11.8 million deaths worldwide each year. Alcohol misuse alone accounts for 3 million deaths per year, representing 5.3% of all deaths worldwide. One challenge in studying substance misuse includes its interaction with other mental health issues. Researchers have repeatedly demonstrated high co-morbidity of substance misuse with both anxiety and depression, but many other psychological risk factors have been observed, including impulsivity, trauma, experiential avoidance, craving, difficulties regulating emotions, and negative affect. Whilst these may look like a divergent set of variables, they all share a similar component: avoiding mental and emotional pain. In contrast to avoidance, mindfulness involves staying present with thoughts, feelings, and bodily sensations. It"s not surprising, therefore, that researchers have found mindfulness-based interventions to be effective for treating substance-based disorders, and studies have identified several mechanisms of change, including emotion-driven impulsivity, negative affect, acting with awareness, emotion regulation, and craving.

- IMPETUS: Investigating Markers derived from Proteomics for Estimation of Transition from the Ultra high risk State to psychotic disorder
- Summary
- Early identification and intervention are associated with improved outcome for psychotic disorders such as schizophrenia. This is facilitated by targeting people at clinical-high-risk (CHR) for psychosis. There is growing evidence on the involvement of the immune system and inflammation in psychotic disorders. We have added to the literature by demonstrating that the complement pathway " the blood plasma component of the innate immune system " is dysregulated in blood samples of individuals at age 11. This is many years before individuals report symptoms in keeping with psychotic disorder (PD) at age 17. Based on our own plasma proteomic data and machine learning analyses we have evidence for a set of biomarker proteins which can predict transition from CHR to PD with a high degree of accuracy. We now seek to refine and replicate these findings. Using state-of-the-art proteomic methods and machine learning approaches we will replicate and refine this biomarker signature of transition from CHR to PD in: three distinct international samples one longitudinal cohort with a total sample size of more than 1,000 CHR subjects. We will also undertake functional assays that measure plasma protein levels of the complement and coagulation pathway and their activation fragments as indicators of complement and coagulation dysregulation.

- Tracking Trajectories of Psychopathology from Infancy to Young Adulthood: an Irish national longitudinal cohort study
- Summary
- Mental illness is the leading cause of disability among young people accounting for nearly half of the overall burden of disease between 10 and 24 years of age. It is estimated that by 2030 mental illness will have a greater global economic impact than any other medical condition. Unfortunately, prevention research into mental illness has lagged far behind conditions such as cardiovascular disease and cancer. 75% of mental disorders have already started in some form before age 25. This developmental timing of onset of mental disorder in youth (unlike other illnesses ) means that there is a disproportionate effect on social, educational and vocational outcomes and contributes to the global economic burden. There is an urgent need for a new approach to tackle mental disorder among young people but there is a paucity of evidence about the etiological mechanisms and presentation of mental illness in youth. Current diagnostic systems are based on adult disorder but early presentations are often undifferentiated and don"t correspond to classical diagnostic systems. This poses challenges for early intervention. This current proposal seeks to investigate, in a large population-based cohort, the presentation, etiological mechanisms, developmental course and outcomes of psychological symptoms in youth. The Growing Up in Ireland study has data on almost 20,000 young people in Ireland in two related cohorts who have been followed up over 12 years. This cohort is the result of a large investment by the Irish government. We will use advanced statistical techniques to model the clustering and progression of psychopathology from early childhood to early adulthood. We have a detailed a progressive dissemination plan to communicate the results of our findings to young people, the public, service providers and policy makers. In this way the information will benefit the lives of young people not just in Ireland but internationally.

- Using gamified mobile dry-EEG technology to detect the acute and delayed effects of methylphenidate on brain activity and cognition in an ADHD population
- Summary
- Children and adolescents with Attention Deficit Hyperactivity Disorder (ADHD) are often treated by administering a stimulant medication " methylphenidate (MPH - Ritalin is a common brand). However, the response to this treatment is highly variable. Currently, there is no way to predict for who this treatment will be effective. We propose to address this problem by utilizing a new sophisticated methodological approach of repeated cognitive testing coupled with large scale brain recordings via electroencephalography. In this project, participants will take portable dry EEG headsets in the home and remotely record their own brain activity while playing gamified cognitive tasks. We will then interrogate brain and behavioural data using machine learning with the goal of identifying the variables that will predict treatment responsiveness.

- Developing a novel intervention to target addictive behaviours by improving inhibitory control through EEG-based brain-computer interface
- Summary
- Response inhibition is the ability to flexibly suppress unwanted or inappropriate actions, and it is a core component of healthy executive control. Impaired response inhibition is characteristic of a wide range of psychiatric diseases and of normal ageing. The underlying neural mechanisms of response inhibition have been researched intensively over the past two decades. It is hypothesised that the right inferior frontal cortex (rIFC) plays a key role by inhibiting the motor cortex via the basal ganglia and thalamus. It is thought that communication in this network is facilitated through Beta oscillations. While the vast majority of literature has investigated averaged Beta power, recent literature suggested that Beta oscillations are characterised by short-lasting, transient `Beta bursts", and only appears to change in sustained amplitude if it is averaged over many trials. However, the detailed functional roles of Beta oscillations and the involved anatomical structures in response inhibition are still unclear. Studies using bigger sample sizes as well as different experimental and analysis methods are necessary to identify generalisable findings which can meaningfully inform potential therapeutic interventions and impact their success. Thus, the overarching aim of this thesis was to advance the existing knowledge of the role of Beta oscillations in response inhibition by using methods designed to establish predictive and causal relationships.

- Note: the original title of the grant was 'Effects of a mobile biofeedback device for stress management' Age-related cognitive decline is an increasingly important societal issue, given projected increases in the proportion of older adults in the coming decades. Early identification of cognitive decline will enable earlier interventions which have a greater likelihood of slowing decline, maintaining quality of life, and reducing the burden on caregivers and society. The application of machine learning to neuroimaging data is a promising strategy to detect age-related cognitive decline. However, there has been little emphasis on the development of measures of two related, yet separable, constructs " brain maintenance and cognitive reserve " both of which support cognitive function as we age. Accurate measurement of these two constructs may improve our ability to detect age-related cognitive decline.

- Identifying individualised EEG biomarkers of mild cognitive impairment (MCI) in older adults using a novel gamified approach and wireless EEG
- Summary
- Background: Mild Cognitive Impairment (MCI) is a common condition in older adults marked by a deterioration in cognitive function greater than expected for an individual"s age. It is regarded as a prodromal stage for dementia; 20-40% of all MCI patients going on to develop the disease. Early detection of MCI is therefore of vital importance for improving diagnostics and preventative care strategies for individuals at increased risk for dementia. The problem: The identification of neurological and cognitive makers that predict MCI has been hindered by methodological and analytical limitations. Namely, neurophysiological data is highly sensitive and suffers from poor signal-to-noise ratio. Further, status-quo statistical methods cannot provide translational measurements that indicate an individual"s likelihood of developing MCI. Project rationale: Recent advances in brain signal measurement and analysis provide new opportunities to address these concerns. Wireless, dry (no electroconductive gel) electroencephalography (EEG), virtual reality gamification and machine learning all represent a new frontier for research into MCI. Aims: Taking advantage of these state-of-the-art methods, we propose to (1) build a neurocognitive profile of older adults at risk of developing MCI and (2) identify individual biomarkers for cognitive impairment in old age. Method: A fully gamified neurocognitive task battery (with 3D graphics and sounds) will be used in conjunction with wireless EEG to investigate cognitive function in normative and at-risk groups of older adults. In addition to group comparisons, data will be interrogated using tailored machine learning algorithms to isolate neural signals that are maximally predictive of MCI. Impact: This project promises to deliver novel insights into the neurocognitive factors underlying age-related cognitive decline. Specifically, results will identify individualised neurological and cognitive makers predictive MCI in older adults. The isolation of these biomarkers also has the potential to inform brain training interventions aimed at preserving cognitive function. In addition, biomarkers may also be employed as a clinical endpoint in future drug trials for MCI/dementia.

- Investigating a digital biomarker of Alzheimer"s Disease using Augmented Reality
- Summary
- Background: Alzheimer"s disease (AD) and dementia affect 46.8 million people worldwide, with an estimated increase of 7.6 million new cases every year [1]. The average annual costs of care per patient are US$9,829 in the U.S.A. and €7,820 in Europe, with annual costs doubling for patients at the severe dementia stages [2]. Due to the high impact that AD has on society, neuro-behavioral research into the core pathogenical processes underlying AD will be imperative to establish prevention methods that decrease or delay the onset of the disease. Current diagnostic measures of AD are invasive (cerebrospinal fluid analysis), expensive (neuroimaging) and time-consuming (neuropsychological assessment) and thus have limited accessibility as frontline screening and diagnostic tools. Aim: This project will develop a novel computational biomarker of AD that can be identified via mobile devices. This technology may facilitate early detection and more effective therapeutic and preventative strategies for AD. Methods: A large sample (N = 60) of at-risk older adults (scoring below the average on age-related cognitive tests and with a first-degree relative with a diagnosis of AD) will be assessed longitudinally (at baseline, 9-month, and 18-month follow-up; Total Assessments N = 180). A comprehensive battery of biological, neurophysiological and cognitive measures will be administered to assay cognitive functioning longitudinally. Assessments will centre on an augmented reality smartphone application (designed by Altoida) that measures spatial navigational within a virtual arena. Outcome and Impact: Given the complex nature of AD pathophysiology, the optimal prediction models for future development of AD may come from longitudinal monitoring of relevant cognitive processes. Due to the demand for early diagnosis at a preclinical and early symptomatic level, this research will ultimately impact clinicians seeking more specific and targeted diagnoses of AD.

- Alzheimer's Disease Prediction Service (ADPS): digital biomarker for Real-World Evidence (RWE) in AD
- Summary
- According to the 2014 Alzheimer report, only 35% of dementia cases are diagnosed early enough. Although experts realize the potential of functional screening biomarkers, such as virtual reality (VR), augmented reality (AR) or activity tracking (AT) technologies in dementia prediction routine examination is not carried out until symptoms appear and significant brain damage is already done. On the other hand, enough data from mobile phone, in-house or driving activity sensors are available today to assess the earliest possible functional and navigation deficiencies and assist early prediction of dementia or other neurodegenerative disease decades in advance. This project builds on previous datasets group, with >2.700 patients and 87-94% diagnostic accuracy of predicting Alzheimer"s up to 6 years in advance. Early information enables prevention. Its objective is to evaluate the accuracy and performance of an ADPS prototype, that continuously tracks everyday cognition, combines different mobile phone activity data layers using machine learning techniques, to estimate the risk of developing Alzheimer"s disease.

As an editor, I triage manuscripts and oversee the review process. I handle manuscripts that cover a broad range of neuroscience research, and editors have acknowledged breadth in recognizing and promoting interdisciplinary collaborations. As part of this role I periodically contribute to the `This week in Science' section in Science Magazine.
I am on the International Advisory Board of the Latin American Brain Health Institute (BrainLat) , which aims to empower innovative brain health research leaders across Latin America, in collaboration with leading centers worldwide.
