Application Process for School of Medicine Research Ethics Committee

1. Please down load the application form (this link does not work) and save on to your computer.
2. Please complete all the required sections on the form.
3. Please save the completed form in the following format only.

[ApplicantnameMonthYear] and as a PDF file. e.g. MaryByrneMay2015.pdf

This file must then:

1. Be sent via email to somrec@tcd.ie before 5pm on the relevant deadline date
2. Be printed by the applicant and signed by the supervisor as listed. This original signed copy must be received by the Secretary to the School of Medicine Research Ethics Committee (Ms. O McBride) before 5pm on the relevant deadline. 
3. The Secretary’s office is in School of Medicine is on Level 1 of the Trinity Biomedical Sciences Institute, TCD.

Notes:

• Any application not meeting either deadline for submission will be automatically rejected
• A cap on the number of applications to be reviewed at any meeting will be 15
• All applications therefore will be marked with time and date on receipt (via email)
